Severity legend

• 🟢 safe: runs automatically, no approval needed
• 🟡 caution: auto-runs by default (depends on permission mode); requires approval in read mode
• 🔴 dangerous: requires approval every time by default; allowlisted per-tool in trust mode; auto-runs in yolo mode (careful)
